This form lets us know what ensemble's you are interested in being a part of at JHS.

If you want to participate in any activity at JHS, it is required to complete this form. This includes travel with the band to Large Group Contest, Solo & Ensemble Contest, and any All-State events.

Parent Forms

This form gives us the opportunity to learn more about you (parents) and give you the chance to let us know how you may want to help your son/daughter during their time in band.

This form must be completed out and submitted before being approved by the district to volunteer/chaperone at and JHS activity.

It is the expectation that students must travel with the class/group/team to and from the event in whatever transportation has been arranged by the school. In extenuating circumstances, a student may travel from the event with his/her own parent/guardian. This form must be completed and returned to the Mr. Macklin or Mr. Kurtz by 4:00 PM the day prior to the departure for the event, for school approval. |
